Service Description: Provides comprehensive assessment, rehabilitation/treatment and supportive counselling to clients with complex and/or serious and persistent mental health disorders, living in North Halton * interprofessional staff include psychiatrists, nurse practitioners, dietitians, social workers, community support workers, child and youth counsellors, occupational therapists, teachers, peer support, etc * referrals are made by doctors through One-Link * services are also offered to older persons with mental health issues, including behavioural and psychological complications associated with neurodegenerative diseases

Services provided include:
  • community-based psychiatric assessment, treatment, counselling and case management for clients and their family members
  • help with understanding and managing medication
  • concurrent disorder treatment/groups (Health and Hope partner)
  • linkages to other health services and community supports



Programs provided throughout Halton Region in partnership with other service providers include:

Child and Youth Psychiatry Program
  • psychiatric consultation services to family doctors for children and youth between the ages of 6 and 18 and to their families who live in North Halton
  • counselling and treatment
  • offered in partnership with ROCK (Reach out Centre for Kids)

Phoenix Early Intervention Program
  • provides family therapy, nursing, individual counselling, psychiatry, peer support and family education for adults and youth living in North Halton
  • clinical assessment and treatment for individuals aged 14-35 years old who are experiencing early signs and symptoms of psychosis

HOMES Program
  • provides clinical case management support and psychiatry for adults living in North Halton
  • clinical support for people living with a mental illness and at risk for homelessness
  • offered in partnership with the HOMES Program

Concurrent Disorders Program
  • provides 10 week psycho-educational support group for people experiencing any combination of  substance misuse, gambling and other mental health issues
  • offered in partnership with the Health and Hope Program

Adult Psychiatry and Treatment Program (age 19 and up) 
  • psychiatric assessment and treatment
  • counselling and treatment
  • offered in partnership with other community partners

Fees: Medical services are covered by OHIP
Application: Individuals seeking services are asked to first discuss their concerns with their family doctor and if a referral is required to have their doctor make the referral on their behalf to One-Link
where there is no family doctor, staff will assist someone in locating a doctor to work with
all referrals require the expressed consent of the person being referred for service
