Service: Daily 24 hours
Service Description: Provides a range of non-medical, assisted living services to support high risk seniors with complex needs to continue living independently in the comfort of their own homes, including personal hygiene activities (washing, bathing, hair care, mouth care, preventative skin care, transferring/ positioning, etc.), personal routine of activities of daily living (dressing/undressing, toileting, help with eating, medication reminders), and essential homemaking services and safety and reassurance checks * services are scheduled at multiple times throughout the day or overnight according to clients' needs and preferences * each scheduled time is customized to the client's needs, depending on the level of support needed and the activity involved

SDL services are designed for seniors who need shorter, more frequent visits to help prevent premature placement in long-term care * clients may be eligible for up to 1.5 hours of personal support each day

Types of services provided:
  • Designated buildings (Hubs): PSWs are located onsite and deliver services to eligible seniors in their individual units, apartments or condos within the building
  • Designated neighbourhoods (Hub and Spoke): In designated neighbourhoods, the Supports for Daily Living service provider has an onsite office in a building that services seniors who live there, but also provides services to eligible seniors who live in neighbouring apartment buildings, town homes, condos or homes within close proximity to the building
  • Mobile service: focuses on getting high risk seniors who are being discharged from hospital home quickly regardless of where they live * SDL service providers travel throughout the region to deliver services until seniors are stabilized at home, then transition seniors to other community partners including other Supports for Daily Living service providers, Ontario Health atHome, or a combination of services that can address a senior’s needs

Eligibility / Target Population: Seniors * insured under OHIP * demonstrates a need for intermittent Personal Support/Attendant Services provided throughout a 24hr period * InterRAI-HC or InterRAI-CHA Assessment MAPLe score of 3 or higher * personally able to direct their own care and communicate their needs (with or without aides) * medically stable and pose no risks to themselves or others * able to be left alone between visits * agreeable/compliant with their service plan * personal care/support required equals approximately 1.5hrs per day
