Service Description:

Complex Continuing Care (CCC) is for patients with complex medical conditions who require ongoing support before transitioning to their next stage of care * patient’s condition may be the result of a chronic or devastating illness marked by impairments in either or both mental and physical function * level of support that patients need ranges from significant to total * CCC is a transitional step in a patient’s care journey helping them recover, rehabilitate, or manage their health before moving to a more permanent location that could be home, a long-term care home, a retirement home, or another setting * complex care beds, long-term rehab beds, and transitional care beds offered * CCC is for patients with multiple chronic complex medical conditions or diagnoses whose needs cannot be met in any other community or residential setting, including long-term care

McCall Centre For Continuing Care
- 120-bed complex continuing care centre (73 inpatient beds providing specialized care for patients who require complex continuing care including end-of-life and palliative services and 47-bed unit providing Activation services, which are also called Slow Stream Rehabilitation) * consists of 21 interim long-term care beds (residential care for hospital patients waiting for long-stay admissions) * 24-hour nursing and personal care * access to a doctor and other health professionals

Moir Family Centre for Complex Continuing Care
- 39 inpatient complex continuing care beds * provides care for patients with highly complex medical needs who require significant assistance with daily activities and may rely on medical technologies such as tracheostomies, feeding tubes, nephrostomy tubes, IV pumps, catheters, special pain management or have complex-wound care needs * single bedrooms to support greater privacy

- The Moir Family Centre for Complex Continuing Care provides 39 inpatient Complex Continuing Care beds * focus of the Moir Centre is on highly complex post-acute complex continuing care * patients at the Moir Centre typically require a significant amount of support for activities of daily living, and may require medical technologies to support their care (e.g. tracheostomies, feeding tubes, nephrostomy tubes, catheters, ventilation, special pain management, etc.)
- The McCall Centre has 73 inpatient beds that provide a range of Complex Continuing Care services including end of life and palliative services * the Centre also has a 47-bed unit providing Activation services, which are also called Slow Stream Rehabilitation * patient in Slow Stream Rehabilitation program needs a less intense, differently paced program than is found in the high intensity rehab beds to enable their recovery * teams will work with each patient to identify and work towards recovery goals, and will discuss transition plans to the next setting with each patient and their family
- Through a partnership with Runnymede Healthcare Centre, Trillium Health Partners also offers Slow Stream Rehabilitation patients access to 33 beds at Runnymede.
