Service Description:
|
Offers treatment and management for individuals whose diabetes and related conditions require a more intensive and comprehensive team-based approach * helps to stabilize and control patient's diabetes and diabetes-related complications such as chronic kidney disease, cardiovascular disease, diabetic retinopathy, neuropathy and related conditions * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * in addition to case management, the Centre also provides patients and their families with care navigation, individualized care planning, frequent follow-up/monitoring, and transition/discharge planning * team consists of a nurse practitioner, registered nurse, registered dietitian, social worker, and pharmacist- treatment and management of related conditions
- prolonged diabetes and comorbidities such as organ failure, frequent hyper or hypoglycemia, foot and skin issues, mental health issues and inability to cope
- psychiatrist consultation
- wound care specialists
- assist in navigating health care system
- referral to diabetes education program or other related community-based support programs, as needed

Eligibility / Target Population:
|
Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes
Older adults and have poor glycemic control with conditions that negatively affecting their diabetes care such as COPD, malignancy, mental illness, and complications associated with their diabetes
