Mississauga Halton CCAC introduces 2014/15 Annual Report to the Community

Mississauga, ON - Mississauga Halton
Community Care Access Centre’s 2014/15 Annual Report to the Community - The Power of Care Coordination – Advisor, Advocate and Coach, highlights the key roles our experienced, care coordinators,
play in the lives of their patients.



Each care coordinator is an advisor, health advocate and coach for their patients – and often for families too. They continually monitor their patients’ health to respond - if and - when their patients’ care needs change,” says Caroline Brereton, CEO, Mississauga Halton CCAC.

 

"Every day, they use their clinical judgment and assessment skills to support patients with increasingly complex care needs. Our care coordinators talk with - and listen to – their patients and develop a personalized care plan that is unique for every individual, based on each patient’s health and social needs,
explains Ms. Brereton.

The Mississauga Halton CCAC coordinates in-home and community support services, information and referral, and long-term care placement. It is funded by theMinistry of Health and Long-Term Care through the Mississauga Halton Local
Health Integration Network and serves a population of more than one million
residents in the communities of South Etobicoke, Mississauga, Oakville, Halton
Hills, Georgetown and Milton.The Mississauga Halton CCAC coordinates in-home and community support services, information and referral, and long-term care placement.
